Transition to Adulthood: System Redesign
This planning initiative is designed to create a cohesive and comprehensive system overhaul to ensure that youth moving from foster care, detention, homelessness or with emotional and/or behavioral challenges have all the supports and skills to become productive, contributing citizens.
The goal is to remove barriers to services through the cooperation of community partners who serve youth through foster care, juvenile justice, mental health and homeless systems. In collaboration with key stakeholders in 2005, a framework for a systems redesign is under development and is targeted for completion in July 2006.
On Wednesday, September 13, 2006, ICanMakeIt.org was launched.
