NOMINATE THEM! - Download criteria and form The recipient of the 2008 Public Service Excellence Award will be honored at The Children's Trust Champions for Children Awards Ceremony on Thursday, Nov. 6, 11:30 am – 1:30 pm at the Jungle Island Treetop Ballroom.
General Information:
- The official nomination form that is available for download below or a duplicate of the form must be used.
- Nominations should be typed.
- Employees should be nominated for exemplary activities.
- Additional materials are accepted or used in support of the nomination.
- The award will be made annually.
- Nominations will not be accepted beyond June 27.
- E-mail your entry to ernie@thechildrenstrust.org. Criteria for Selection:
• Nominee is an employee (not a manager or executive director) of a program or initiative funded by The Children's Trust.
• The nominee's primary duties are to provide direct service to children and families.
• Nominee consistently performs above and beyond job description and requirements to serve the children and families of Miami-Dade. (Nominee willingly takes on the extra load, extends a third and fourth helping hand and goes the extra mile.)
• Nominee exemplifies the belief that the human element makes a difference in the lives of others, and recognizes that the human touch must be extended far beyond the confines of a standard workday.
