The files that you will download are compressed and/or "zipped" files containing all the elements you will need to respond. The files are in Adobe Acrobat/PKZIP "pdf" formant and MS Word/OpenOffice "doc" format.
When you download the file you will need to "unzip" the file with a free utility like PKZIP. A link to PKZIP can be found below. A link to Adobe's free Reader can also be found below.
Forms are provided in MS Word/OpenOffice documents so that you may fill them in on a personal computer, or print them out and fill them in.
